Dr. Leonard, a founder of Safe & Reliable Healthcare, is a high reliability expert with a deep focus on safety culture and teamwork in high-risk environments and in diverse areas of clinical practice. Boarded in anesthesia and Internal Medicine, Dr. Leonard spent two decades with Kaiser Permanente, including 10 years as the National Physician Leader for Patient Safety across the Kaiser system. In 1999, he helped Kaiser forge a collaborative relationship with Dr. Robert Helmreich’s Human Factors Research Project, which was seminal in bringing Crew Resource Management into aviation, to work on the application of human factors teamwork and communication training into healthcare. This collaborative also led to the development of the world’s first healthcare culture survey with Professor Bryan Sexton.

Dr. Leonard serves as a longtime co-editor of the Joint Commission’s Essential Guide for Patient Safety Officers along with Dr. Allan Frankel. Key tenets of the central framework for high reliability that Safe & Reliable Healthcare co-developed and co-published with the Institute of Healthcare Improvement, including those related to assessment and change management, have been adopted by various organization’s including the ORO 2.0 program. Additionally, Dr. Leonard has been active in several domains at the IHI including the Patient Safety Officer Training Course, Transforming Care at the Bedside, the Safer Patients Initiative in the United Kingdom, and Patient Safety Scotland.

Dr. Leonard is the senior consultant and advisor to the Military Health System where his project team has designed, and is leading, their high reliability transformation under MG Place. Previously, Dr. Leonard was on the Department of Defence’s TeamSTEPPS Development Committee.
